The Chronic Disease Management GP services enables you Doctor to plan and coordinate your health care. If you have a chronic disease your GP might think you require a multidisciplinary, team-based care approach from a GP and at least two other health or care providers.

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions, mental health issues and stroke.

Patients who have a chronic medical condition and complex care needs and are being managed under a GP Management Plan and/or Team Care Arrangements are also eligible for Medicare rebates for certain allied health services with referral from their GP.

Speak with your GP at the Coffs GP Super Clinic today about how a Chronic Disease Management plan might help you with the long term planning for your health.
